Homeschool Fall Field Day takes place Saturday, October 31, 2026, 9:00–11:00 a.m. Central, at Life Time Frisco, 4900 Throne Hall Way, Frisco, TX. Event organizations: The Athletic Collegiate LLC; Life Time; Texas Sports Academy. The Eventbrite RSVP, this TAC event form and Life Time’s guest admission are separate steps. This form does not reserve an additional ticket or certify Life Time admission. I am at least 18 years old and sign for myself. For every child I list, I am that child’s parent or legal guardian and have authority to give participation permission and accept the required photo and video release for that child. I do not sign for any other adult. Each additional adult must complete their own form and Life Time’s required guest agreements.

Planned activities include tennis, soccer, supervised archery, running games and relays, team challenges, an inflatable obstacle course, and face or pumpkin painting. Activities, equipment and the schedule may change for safety, weather, staffing or venue requirements. Sport, projectiles, running, collisions, falls, inflatables, equipment, heat, fatigue, paint and food exposure can cause injury or allergic reactions; serious injury or death is possible. I will consider each child’s abilities, follow equipment and instructor rules, report a safety concern and stop participation when needed. I can request support privately without providing a diagnosis on this form. Food and amenities are subject to Life Time’s rules and the event’s confirmed arrangements. I have read the event participation and risk information, give permission for each listed athlete to take part in the activities I consider appropriate, and will follow the staff’s instructions. I may decline an activity or ask my child to stop at any time.

A parent or legal guardian must remain on-site, be reachable and supervise their children between activity stations. This event is not a drop-off childcare service. Athletes take part only in staff-directed, age-appropriate activities; spectator siblings are listed for attendance and permissions and are not registered for the athlete rotations. Each family follows Life Time’s guest and age rules. If an urgent incident occurs, event staff may contact emergency services and the emergency contact I list, and provide first aid within their training. This is permission to seek help, not a promise of medical coverage or a blanket authorization for non-emergency medical treatment. I remain responsible for decisions and costs as provided by law.

Photography, video and audio recording will take place at this event. Acceptance of this photo and video release is a condition of entry and participation for every attendee. By signing this form and entering or participating in the event, I accept this release for myself and each child I list as their parent or legal guardian. I authorize recording of our images, likenesses and voices during the event and grant The Athletic Collegiate LLC; Life Time; Texas Sports Academy permission to use them in event recaps and in their program and event promotion, including websites, social media, printed materials and paid advertising. This permission is worldwide and royalty-free; no payment or final approval is due. Reasonable editing is permitted, but it must not misrepresent an endorsement. No child’s full name, school, contact information or private registration details may be published under this release. Use is limited to the organizations named in this signed record; adding a sponsor later does not grant that sponsor these rights. If I do not accept this release for myself or a listed child, that person must not enter or participate in the event. Every additional adult must accept the release in their own signed form. I may contact admin@theathleticcollegiate.com about withdrawing permission for future use. TAC will record the request and notify the named recipients; previously printed or distributed materials and third-party reposts may not be fully retrievable. A withdrawal before entry does not authorize attendance without the required release. This release does not authorize recording in private areas or override Life Time’s photography rules.

I understand TAC will use these details to administer this event, record my permissions, contact me about attendance or support, and provide my receipt and event instructions. TAC may provide Life Time and Texas Sports Academy with the limited adult contact details, attendee names, ages and registration statuses needed for this event. Emergency contact details are restricted to staff coordinating safety or emergency response. Other sponsors do not receive our family’s private registration, signatures or emergency contacts. Life Time collects information directly under its own privacy policy when I complete its guest registration. TAC’s Privacy Policy is available at /privacypolicy. Signed event records are retained for event administration, safety, legal and claims-handling needs, with access limited to authorized staff. Request access, a correction, a withdrawal or deletion by contacting admin@theathleticcollegiate.com; records needed to document a prior permission, handle a claim or comply with law may need to be retained. Future marketing choices are separate.

I have read the displayed agreement and my selections. By typing my full name and submitting, I intend to sign this event record electronically. I agree to use an electronic record and can download a copy after submission. I can request a paper signing option by contacting TAC before the event.

Optional: for each organization I select, I would like to receive emails about its programs and future events. TAC may share my adult name and email address with that selected organization for this purpose. My children’s details, signature, phone number and emergency information are not included. This is not required to attend or to obtain the event guest access. I can unsubscribe from each organization’s emails.
